Membership contract and general (payment) Conditions: The present General Conditions apply to the contract signed between you and Existence S.A.R.L.. In signing this subscription form, 2 pages contract, you declare to accept the present General Conditions which will have been explained to you by one of Existence’s staff and having received a copy of this, by both parties signed, subscription contract. Membership 1. Membership starts always on the first day of the coming month, and lasts for either 3 months (basic ), 12 months (result) or 24 months (success). If you subscribe before the first day of the new month, the remaining X days of the month will be added to the first payment term. To be calculated as ((monthly price/30) * X ) . Existence will charge you automatically from the 1 of the new month your payments for a 12 or 24 month membership. A 3 month membership has to be paid at first visit each new period at the reception. Together with this payment you will be charged one month ‘’guarantee’’ fee. This month’s membership fee (after an accepted membership cancellation), is meant for the last training month, added after the contract period of 12 or 24. 2. Unless notice of termination is given by you one month before the subscription’s expiry date, your 12 or 24 month-membership will automatically be renewed for an undefined duration, as from the first day of the following month. You can than cancel your monthly membership. Your 3 month-membership will automatically be renewed for 3 months. If you do not give notice of termination, you remain liable to pay. There will be no refund of the remaining months. 3. In order to cancel this contract you have to either write a letter of resignation or fill out the “exit-form”. This form is available at the reception. It should be filled out, signed and posted. The copy of this completed and signed form serves as proof that Existence has received your application to cancel the contract. We therefore advise you to send in your letter of resignation or the ‘’exit-form’’ by registered post addressed to Existence. Administration date is the date of the post office tamp. Only a written notice of termination will be considered by the administration. The one month deadline before the expiry of your program will be strictly applied. 4.
